I have a civil case at EDCA,the clerk has issued a certificate of entry of default. Now do I have to file a motion for default judgement?

I have these questions:

1.Is there a form or format for motion for default judgement?

2.Should I propose a hearing date for default judgement in my motion?

3.Should I attach an affidavit to my motion indicating how relief was calculated and if so is there a form or format for such affidavit?
