I was adopted 3-4 years ago. Now that I'm 19 I want my biological father's last name and him put back on my birth certificate. Both of my parents are dead. How or can I get this done?????

Answer

If your biological father is still alive, he can adopt you. If he is dead, then there is nothing you can do other than request a name change.
